Heart Failure Program

Sanford's Heart Failure Program is for those diagnosed with heart failure. The program educates patients about the condition and helps them manage their symptoms to live a longer, more comfortable life.

The Heart Failure Program features a team of:

  • Cardiologists
  • Primary care physicians
  • Nurse practitioners
  • Nurses

Together, the Heart Failure Program Team will work with the patient to offer one-on-one consultations, educational resources and a personalized treatment plan.

Mission: To promote an improved quality of life for patients and their support systems, beginning with service through excellence.

**This program is for those diagnosed with heart failure. Patients cannot enroll themselves. A primary care physician or cardiologist may refer patients to the program by calling 701.323.5202 or 800.932.8758.
